Routine Checkups For Children

The Children’s Clinic of Nashville is committed to keeping your children healthy. An essential part of maintaining good health is the routine well-child visit. Also, we discuss age-specific health, behavior and safety issues as well as determine your child’s readiness for participation in physical activity, from playing outdoors to competitive sports. Most importantly, the Well Visit is an opportunity for you to share any concerns you may have regarding your child’s health, and for us to get to know you and child better. Both contribute to better wellness overall, and we look forward to these visits!

Important Paperwork During Your Pediatric Visit

At the conclusion of every Well Visit, you will be provided a current immunization record as well as a completed “Camp/Sports Participation Form†for your convenience. These forms are also available on the portal following the checkup. At other times of the year, we can also complete any additional school health forms, camp forms or sports forms as long as your child’s well-child visit is current (within the last 12 months for children two years of age and older). It is not necessary to go to urgent care or walk-in clinic for a sports physical.

To avoid the angst of school and camp deadlines in the summer, we advise you to schedule your child’s annual visit in the month of their birthday each year so that when forms are due, we are able to meet those deadlines easily.
